Please select and complete the forms listed below for initial program enrollment!

139-R – ROTC Program enrollment application. Only complete the first two pages. Sign at the bottom of page two. But send back all 6. ***PLEASE TYPE IN THIS INFORMATION INTO THE BOXES ON THIS FORM!!!*** It makes things a lot easier once this document is uploaded in the system.

136-R – This is in case you suffer illness/injury as a result of authorized (scheduled and supervised) training, or authorized travel to and from such training, are eligible for compensation through the Department of labor/Department of Veteran Affairs.

137-R – This form allows us to view grades, transcripts, and academic records.

DD 2005 – This allows us to view your medical information, imitate your DODMERB physical, and submit waivers on your behalf if needed.

DA 3425 – This is just a standard form that green lights your ability to participate in things such as labs and PT (This must be signed by a physician).
